Feedback Forms
The feedback forms page lists the post-appointment surveys your organisation uses to collect responses from clients.
Overview
This page is available when the Feedback module is enabled for your organisation.
Searching
Use the search bar to find a form by name. Forms are listed alphabetically and show their purpose, scope, validity, an anonymous indicator, and the number of questions.
Purpose
Each form has a purpose, shown as a chip in the list: Visit, Booking experience, Event, Cancellation, No-show, Campaign, or Relationship.
Scope
Each form is either organisation-wide or limited to specific resources. The scope column shows "Organisation-wide" or the number of resources the form applies to.
Creating and Editing Forms
- Click the Add button and choose Blank form to start from scratch, or From a template to duplicate one of the ready-made system templates (this option is only available once system templates exist).
- To open an existing form, double-click its row, or click the view icon.
- In the form editor, set the name, purpose, scope, validity dates, an anonymous option, and the questions clients will answer, then save.
Click the duplicate icon on a form to create a copy of it, named after the original with "(copy)" added, that you can then edit independently.
Creating, editing, and duplicating forms requires configuration write permission. Without it, the Add button and the duplicate icon are hidden and the form editor opens in read-only mode.
Viewing Responses
Click the responses icon on a form to open its responses page, where you can browse and read the submissions collected for that form.
Feedback Insights
Click the chart icon in the top right to open Feedback Insights, where you can review aggregated results across your feedback forms.
